This form can be used to register your team for the 2009 MHCI 9 event held in Las Vegas on October 26-27. This form should be filled out by the person designated as the team captain and only needs to be submitted once per team. To secure your foursomes spot in the MHCI 9, please send a goodwill deposit of $5,000 to the Tournament office by August 25, 2009 and the balance by October 5, 2009.
